Wisconsin DSPS Forms 

Wisconsin DSPS is no longer processing the paper/PDF forms for Notice of Employment, Notice of Termination, etc.  Please logon to your brokerage entity with DSPS and use Amendments -> Request for Amendment to find, complete and submit the appropriate amendment online for processing.  See video tutorials at: https://dsps.wi.gov/Pages/LicensEResources.aspx

After submitting an amendment online, go to Amendments -> My Amendments to view the pending amendment and please print a copy to PDF to forward to GNMLS for new or terminated agents.  It may print across multiple pages, but that's fine - we just need a copy as proof that it was submitted to DSPS for processing in order to complete your agent addition or termination on our end.
